The Highland Marketing advisory board consider the huge agenda he is now facing, and what it could \u2013 and should \u2013 mean for health tech. <\/strong><\/p>\n<p>Matt Hancock\u2019s downfall as health and social care secretary was swift. Last week, barely 12-hours after he had delivered the keynote speech to the NHS Confederation\u2019s annual conference, <a href=\"https:\/\/www.thesun.co.uk\/news\/15388014\/matt-hancock-secret-affair-with-aide\/\">The Sun newspaper published<\/a> a CCTV grab of him enjoying a \u201csteamy clinch\u201d with an aide.<\/p>\n<div id=\"attachment_5990\" style=\"width: 310px\" class=\"wp-caption alignleft\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-5990\" class=\"size-medium wp-image-5990\" src=\"https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Javid-300x200.jpg\" alt=\"\" width=\"300\" height=\"200\" srcset=\"https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Javid-300x200.jpg 300w, https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Javid-768x512.jpg 768w, https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Javid-150x100.jpg 150w, https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Javid-400x267.jpg 400w, https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Javid.jpg 1000w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><p id=\"caption-attachment-5990\" class=\"wp-caption-text\">Sajid Javid<\/p><\/div>\n<p>On Saturday, despite the palpable reluctance of prime minister Boris Johnson to sack him, he was <a href=\"https:\/\/www.bbc.co.uk\/news\/uk-57625508\">forced to resign (BBC News)<\/a> as the police started an investigation into whether he had broken the law when he breached his own measures to control the spread of Covid-19.<\/p>\n<p>By the end of the day,\u00a0<span style=\"font-size: 14px;\">Sajid Javid a\u00a0<\/span>former investment banker with Cabinet experience as culture, business, and home secretary and, briefly, chancellor, had <a href=\"https:\/\/news.sky.com\/story\/pm-appoints-former-chancellor-sajid-javid-as-new-health-secretary-12342685\">been announced as Hancock\u2019s successor (Sky News)<\/a>.<\/p>\n<p><strong>A huge agenda <\/strong><\/p>\n<p><a href=\"https:\/\/www.highland-marketing.com\/event-coverage\/a-huge-task-ahead-nhsconfed-nhsreset\/\">Confed underlined the huge agenda<\/a> that is facing health and care over the next five-years, and which is now sitting in Javid\u2019s in-tray. Immediately, the NHS needs a new chief executive to replace Sir Simon Stevens, who is leaving after seven years.<\/p>\n<p>It needs to see the detail of the legislation that will tear up the Lansley reforms of 2012, deliver significant new powers to the Department of Health and Social Care, merge NHS England and NHS Improvement, and create integrated care systems and integrated care providers.<\/p>\n<p>At the frontline, it needs to get through the remainder of the Covid-19 vaccination programme, to navigate another wave of the disease plus flu this winter, to get stuck into a waiting list recovery programme, and to prepare for that shift to integrated care and population health-management.<\/p>\n<p>Meanwhile, social care desperately needs the long-term fix promised by prime minister Boris Johnson as he took office. And both health and social care need to be able to offer something to an exhausted workforce upset at being offered a real-terms pay cut.<\/p>\n<p>Imaging expert Rizwan Malik argued that \u201cworkforce planning has got to be central to Javid\u2019s strategy for the NHS\u201d because \u201cwe won\u2019t be able to address the waiting list crisis or anything else if we don\u2019t address chronic staff shortages.\u201d<\/p>\n<p>As part of that, he added, the new health and care secretary should stop the attacks on \u201cforeign\u201d staff triggered by would-be NHS England chief executive Dido Harding (<a href=\"https:\/\/www.thetimes.co.uk\/article\/dido-harding-make-nhs-less-reliant-on-foreigners-0t6mq9w2b\">The Times<\/a>), and reign in criticism of general practice, which is struggling to find a balance between face to face and remote working post-pandemic.<\/p>\n<p>Jeremy Nettle, the chair of Highland Marketing\u2019s advisory board, pointed out on Monday morning: \u201cSimon Stevens is leaving in 30 days; it\u2019s going to be quite a handover.\u201d<\/p>\n<p><strong>Start by heading off the looming leadership crisis <\/strong><\/p>\n<p>Andy Kinnear, a consultant with 30-years of experience in the NHS, felt Javid should start by addressing the imminent leadership crisis that will be caused by Sir Simon\u2019s departure. \u201cThis is a leadership challenge,\u201d he said.<\/p>\n<p>\u201cSimon Stevens has done a pretty stellar job in the face of some tough austerity measures, and over the past 18-months we have seen some great leadership from figures like Jonathan Van-Tam [the deputy chief medical officer] and Chris Whitty [his boss]. What they all have in common is a deep commitment to and understanding of the NHS, and we need more of the same.\u201d<\/p>\n<p>A number of political commentators have pointed out that Hancock\u2019s departure could be bad news for Harding, the former Talk Talk executive and Test and Trace leader who has been publicly lobbying for his job \u2013 to the <a href=\"https:\/\/www.hsj.co.uk\/commissioning\/dido-harding-applies-to-be-nhs-england-chief\/7030331.article?utm_source=t.co&amp;utm_medium=Social&amp;utm_campaign=newsfeed\">disgust of many managers (Health Service Journal)<\/a> and medical professionals.<\/p>\n<p>But if Javid is going to look beyond Harding and the limited list of other names in the frame, he will need time. Entrepreneur Ravi Kumar suggested Sir Simon might be asked to stay on, or an interim leader asked to step in for six months or a year.<img loading=\"lazy\" decoding=\"async\" class=\"alignright wp-image-5991\" src=\"https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Highlamd-medical-1024x581.jpg\" alt=\"\" width=\"450\" height=\"255\" srcset=\"https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Highlamd-medical-1024x581.jpg 1024w, https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Highlamd-medical-300x170.jpg 300w, https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Highlamd-medical-768x436.jpg 768w, https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Highlamd-medical-150x85.jpg 150w, https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Highlamd-medical-400x227.jpg 400w, https:\/\/www.gpsj.co.uk\/wp-content\/uploads\/2021\/07\/Highlamd-medical.jpg 1359w\" sizes=\"auto, (max-width: 450px) 100vw, 450px\" \/><\/p>\n<p>Nicola Haywood-Alexander, group CIO at NHS Lincolnshire, added that transparency in the recruitment process was hugely important. \u201cWe need a leader with integrity, who understands what it is like to work at the frontline,\u201d she said.<\/p>\n<p>\u201cPeople need to have trust in whoever is leading the NHS, and in the appointment process itself, because inevitably questions will be asked and candidates scrutinised. When I am with our health and care staff, you can see and hear who has their fellowship.\u201d<\/p>\n<p><strong>Then stall the legislation? <\/strong><\/p>\n<p>If he wants to get on top of his in-tray, and then make his own mark on health and care, Javid may also need to stall the legislation required to implement the <a href=\"https:\/\/www.gov.uk\/government\/publications\/working-together-to-improve-health-and-social-care-for-all\/integration-and-innovation-working-together-to-improve-health-and-social-care-for-all-html-version\">Integration and Innovation white paper<\/a>. This will have to come forward soon, if ICSs are going to start work on a statutory basis next April.<\/p>\n<p>But some political correspondents have been reporting that Number 10 is worried about the timing, the size of the changes proposed, and the plans to hand much more control over to ministers (<a href=\"https:\/\/inews.co.uk\/news\/matt-hancock-nhs-reform-opposition-1068228\">The Independent<\/a>). If the legislation is delayed, however, it will raise the question of what Javid wants to do with his new brief.<\/p>\n<p>As a young minister, he had close links with think-tanks such as the Institute for Economic Affairs, which is associated with calls for a smaller social safety net, smaller state and more private involvement in healthcare, and in his brief stint as chancellor he continued his predecessors\u2019 policies of austerity and public service cuts (<a href=\"https:\/\/www.independent.co.uk\/news\/uk\/politics\/matt-hancock-sajid-javid-covid-health-secretary-b1873502.html\">The Independent<\/a>).<\/p>\n<p>Haywood-Alexander mused: \u201cJavid has a good economic background. He\u2019s got Cabinet and finance experience. But health is a tough gig, with huge demand, backlog, and costs, and the well-being of our staff and the ongoing lack of workforce across health and care need tackling.<\/p>\n<p>\u201cAlso, where does he stand on privatisation? Or, more importantly, on priority of esteem between physical and mental health? Or social care? Or health inequalities? I\u2019m not so worried about his capacity to do the job of a minister, but I do wonder what direction he will take, and how he will prioritise and tackle investment in these challenges.\u201d<\/p>\n<p><strong>Refresh the digital agenda \u00a0\u00a0\u00a0\u00a0<\/strong><\/p>\n<p>Hancock tried to put his mark on health and care by focusing on technology and imaging expert Malik argued that his successor needed to do the same. \u201cDon\u2019t just do the typical political thing of reversing stuff your predecessor did, because you didn\u2019t start it,\u201d he advised.<\/p>\n<p>However, many members of the health tech community would argue that Hancock didn\u2019t always focus on the right things. Many hospitals still lack electronic patient records, ICSs have only just been told to get a \u2018basic\u2019 shared care record in place, and the NHS and Covid-19 apps have struggled.<\/p>\n<p>In addition, NHSX, the agency that Hancock set-up with considerable fanfare, has failed to find a clear role alongside the policy making activity of the DHSC or NHS England or the delivery functions of NHS Digital, while many NHS digital leaders feel it has distorted local priorities by focusing on headline grabbing initiatives.<\/p>\n<p>Cindy Fedell, an ex-NHS CIO now working in Ontario, argued it is doubtful that X will survive Hancock\u2019s departure. James Norman, former NHS CIO and now healthcare CIO, EMEA, at Dell, agreed. \u201cNHSX was already being consumed by the new transformation directorate at NHS England, and I think this is likely to signal its end in any form.\u201d<\/p>\n<p>Like Malik, Norman argued the challenge for Javid will be to \u201cmake sure he does not throw the baby out with the bathwater\u201d but to \u201cmake sure the best of NHSX continues\u201d because \u201cthe drive to accelerate change with technology was right, it was just the approach of setting up yet another arms length body to deliver it that was flawed.\u201d<\/p>\n<p>By \u201cthe best\u201d of NHSX, Norman added that he meant its ability to focus attention on important areas such as interoperability, standards and the use of data. Kinnear agreed there is a \u201chuge opportunity\u201d in open architecture and open systems and in Hancock\u2019s <a href=\"https:\/\/www.digitalhealth.net\/2021\/03\/hancock-to-look-at-creating-consistent-cloud-platform-for-patient-data\/\">relatively new commitment to \u201cseparate data from the applications that use it\u201d<\/a> (<a href=\"http:\/\/digitalhealth.net\" class=\"autohyperlink\">digitalhealth.net<\/a>).<\/p>\n<p>This is one area in which Javid\u2019s background might be reassuring. Banks invest large sums in IT and the arrival of open banking standards has driven innovation in fin tech. Nettle argued banks also understand \u201cself-service\u201d; or getting users to do some of the work of checking records, carrying out transactions, and booking services themselves.<\/p>\n<p>\u201cIt\u2019s not for everybody, and we know we have to keep digital inequalities in mind, but we need a digital front-door for the NHS, and as a banker Javid should understand that,\u201d he said.<\/p>\n<p><strong>Time is ticking \u00a0<\/strong><\/p>\n<p>Unfortunately, the NHS digital and data agenda isn\u2019t in a great place with the public at the moment, as a result of <a href=\"https:\/\/www.opendemocracy.net\/en\/tagged\/palantir\/\">Palantir\u2019s involvement in the NHS Covid-19 Data Store<\/a> (OpenDemocracy), NHS Digital\u2019s decision to try and scrape the entire GP record without consultation, and the publication of a data strategy <a href=\"https:\/\/www.digitalhealth.net\/2021\/06\/data-strategy-put-nhs-patients-ahead-of-demands-for-profit\/\">light on privacy safeguards or limitations on future use<\/a>.<\/p>\n<p>The advisory board argued that Javid would be advised to quietly extend the consultation period on the data strategy, and to put off the planned publication of the new NHS digital strategy, \u2018what good looks like\u2019 and \u2018who pays for what\u2019. Or, if he wanted to be really radical, to scrap them and give chief information officers the status and space \u201cto get on and do our jobs,\u201d as Fedell put it.<\/p>\n<p>He may also need to explain or resign from his role as a <a href=\"https:\/\/www.dailymail.co.uk\/news\/article-9001033\/Sajid-Javids-300-000-hustles-Ex-Chancellors-1-900-hour-role-AI-firm.html\">well-paid adviser to C3.ai (Daily Mail)<\/a>, a Palantir competitor. Most of all, though, Javid needs to buy himself time and prioritise that huge agenda; or he\u2019s likely to find himself consumed by it.<\/p>\n<p>Kumar noted that he has already made one rushed decision by promising to lift all the remaining Covid-19 restrictions in England on 11 July. His pledge to make \u201cfreedom-day\u201d \u201cirreversible\u201d will <a href=\"https:\/\/www.dailymail.co.uk\/video\/coronavirus\/video-2448559\/Sajid-Javid-irreversible-Freedom-Day-roadmap-details.html?ns_mchannel=rss&amp;ito=1490&amp;ns_campaign=1490\">please the anti-lockdown tabloids (Daily Mail)<\/a>; but could come back to haunt the NHS in a bad winter.<\/p>\n<p>Fedell suggested seasoned health and care pros would be on the lookout for other, eye-catching initiatives. \u201cHancock came in saying he\u2019d run the NHS with an app. We\u2019ll know pretty soon whether that\u2019s Javid\u2019s style.
